Who gets services at New Horizons
New Horizons is for anyone who:
- Has had surgery or been in the hospital and still needs skilled care before going home.
- Has had an illness or injury and needs a supportive place to recover.
- Needs help with meals, laundry and/or personal care.
Types of stays at New Horizons
New Horizons offers a few options, depending on your needs or the needs of a loved one:
- Short-term skilled nursing and therapies to home. This type of stay is best if you need medical care as you get over an illness, injury or surgery. This could last from days to months — until you’re ready to go home. It includes personalized treatment to help you get back to regular activities.
- Long-term care. This option offers a safe, comfortable place to live with trained, caring people who can help with bathing, laundry, meals and other everyday tasks. Long-term care can last for months or years.
- Respite care. This kind of stay can last from a few hours to several weeks. If you need a break from caring for a loved one, this option lets your loved one get the care they need while you have the chance to rest, travel or get your own medical care.

What to expect at New Horizons
New Horizons supports all patients’ health, safety and well-being — whether you’re here for days or years.
While our New Horizons is inside a medical center and has healthcare experts on site, it is not a hospital. It’s home, for as long as you and your loved one need it.
And because it’s where people do their daily living, we have a Resident Council. This independent, organized group meets monthly to discuss concerns, plan activities and suggest ways to make life at New Horizons the best it can be.
