Infusion Services at Cottage Grove Community Medical Center
Our Services
We provide our community and surrounding areas with hospital-based outpatient infusion services including but not limited to:
- Coumadin/Lovenox bridging
- Therapeutic injections
- Therapeutic Phlebotomy
- Blood Transfusion: Consent signed by ordering physician required. Valid for the duration of the order.
- Central line care
- Limited Biotherapy infusion
- Non-chemotherapy infusion
- IV Hydration with or without electrolytes
- ACTH stim test
- If there are other medications that you would like to order for your patient but do not see them listed in our order sets, please reach out to us and we will do our best to accommodate your needs.
We require written, signed, dated and timed orders, including standard emergency medications to treat potential reactions. Orders valid for up to one year. All orders will be reviewed for accuracy by an RN and a Pharmacist. We will need your contact information so that we can reach you with any questions or concerns.
Please provide us with insurance prior authorization information or documentation that prior authorization is not required. Once we have all of the necessary orders and documentation, we will call the patient to schedule.
We are generally able to accommodate new infusion orders within 1-2 weeks of receiving all the required elements. If the orders are more urgent, please call us to confirm we can accommodate your patient in a timely manner.

How to Refer a Patient
If you are not a PeaceHealth Medical Group provider, the following elements are required before we can process your order and schedule your patient for treatment.
Please provide the following information, with the Cottage Grove Infusion Center intake and order form, to the intake coordinator FAX number 541-767-5399 or E-Fax 541-434-3164.
- Recent progress notes from the ordering provider
- Relevant laboratory results and other supporting documentation
- Insurance documentation with the authorization number or reference number
- Complete orders (please use the PeaceHealth-approved paper orders)
- Blood Consents need to be signed by ordering provider, and are valid for the duration of the transfusion order, until the order is changed, or until the diagnosis changes
